Workers of Lower Manhattan

NYS Workers' Compensation Board 
For case workers to get complete and  up to date information on the status 
of  a client's workers compensation case,  the form OC110A provides 
authorization  to release information to a specific  person.

Once the completed form is mailed to: PO Box 5205,  Binghamton, NY 13902  
Please allow 10 days for processing  after which information can be obtained 
at (800) 877-1373.  * If you are a rescue and recovery  worker (paid or 
volunteer) who did  clean up after 9/11, you must register  now with the 
Workers' Compensation  Board to protect your right to file a  claim if and 
when you become sick. Deadline for  has been Extended to  
September 11, 2010.
